*Cost is per person, non-refundable and does not include your hotel accommodations which have been negotiated for you at a special group rate of $239 per night at Orlando World Center Marriott Resort.  To receive this rate, hotel reservations will need to be made through Pharmacy Development Services.  (Upon receipt of this completed form, Pharmacy Development Services will email you the hotel reservation form.)
