Growth Through Goal Setting (and Diversifying!)
Every year since inception, Cynthia has sat down and written out one or two major goals for the new year. One year the focus was on compounding. Another year they focused on their regular retail section. Cynthia dove in and figured out how to make the most out of the prescriptions they were doing, whether it was with therapeutic optimizations or finding different wholesalers.
This past year Cynthia focused on OTC. It was the one focus she hadn’t done yet, and it made sense that it would be the end of her attention if she’s going to go down the list of things to grow in the business. “OTCs really aren’t super profitable typically; Band-Aids, Advil, some Epsom salts, it’s not how you’re going to pay the bills. So, when I decided to focus on OTCs for 2019, CBD and vitamins were the top of our focus. We’ve really been able to grow that area of our business.”
“Just in 2019, we sold over $215,000 worth of CBD products,” she says with excitement. Cynthia has one employee that is considered their in-house CBD expert. They’ve advertised it more, and she spends a lot of time vetting different companies to make sure she’s putting high-quality products on her shelves. She’s very hands-on and feels it’s a prerequisite for being a successful and honest business owner.
Compounding for Success
Sweetgrass has been compounding from the very beginning. As soon as they first opened, compounding was a moneymaker. “I didn’t know how to make money on regular prescriptions. I focused on compounding and that’s really what drove my success for the first four years of my business.”
Sweetgrass is averaging about 1,200 to 1,300 compounds per month. For a while, compounding equated to 50 to 70 percent of their gross profit margin. It wasn’t until July of 2019 that it balanced out with the prescription side of the business.
They even have their own, full-time veterinary pharmacist, who’s the only board-certified veterinary pharmacist in the state. They do everything from sharks to gorillas to pet monkeys, and then of course, dogs and cats. “That is a large part of our business, which is super fun and always keeps us guessing. You never know what’s going to walk through that door next.”
The other thing that Cynthia enjoys about compounding is working with special needs children. No one else will make the medicine the way these patients need it, and Sweetgrass proudly makes it happen. “Whether they need to be dye-free, sugar-free or made into a suspension so it can go through a G-tube or a J-tube, we do whatever it takes to get done.”
“That’s one of my favorite parts about compounding. We see the change we can make in people’s lives by working with their doctor and adjusting their medications, just a little.”

Creating Change is a Team Effort
Seip Drug has 10 pharmacies, a LTC pharmacy, and two telepharmacies. The PBMs were beating them up in DIR fees, and owner Nate went to the PDS Super-Conference searching for answers. He went home Sunday inspired, he was ready to do it, to create real change in his pharmacy. Unfortunately, Nate faced some health issues but his team stepped in. They joined PDS and started focusing on SmartDispense and therapy optimization where they started to see success. Nate credits his team for coming together and seeing the vision with the help of PDS.
Working Smarter, Not Harder
The team attended the Sync Training which really helped them push their pharmacies forward and create efficiency. They realized that through synchronization, not only were they creating more time to work on the business, they were significantly helping their patients. They truly believe that Sync has helped them reduce DIR fees, improve EQuIPP scores, and most importantly, put their patients first.

Working On Your Business Leads to Growth
Thornhill’s Pharmacy claims that working with PDS allowed them to take a step back and work ON their pharmacy, and it paid off. The success they have experienced has allowed them to transition from scared to excited and also allowed them to open a second location.
Choosing the Right Team
Thornhill’s new location is a “catch-all” pharmacy. It’s going to be traditional retail, compounding, and clinical services as well. They have hired a new PIC that’s going to run the new store; a compounding guru. She’s very intelligent in that field. They have also hired one of the top pharmacists in town to expand their clinical services and perform annual wellness visits. Owner, Jared knows the value of a good team and is excited to be working with such talented individuals.

Stepping Out of the Comfort Zone
Owner, Mary Walsh joined PDS and took a store that had lost money three years in a row to their best year yet. When she was struggling, Mary read PDS Founder, Dan Benamoz’s, white paper Adapt or Go Home. She knew she had to step outside of her comfort zone or she was going to lose everything she had worked so hard for. She didn’t want to let her employees or her customers down. She got out of the store, she took chances on new ideas, and now she is thriving.
Taking it to the Next Level
Because of their success in the first year with PDS, Exton Pharmacy now has enough money to remodel the store and put in a nice consultation room. Exton Pharmacy is fully on board with Service-Based Pharmacy and they are excited (and financially able) to jump into clinical services so they can continue to grow their revenue, outside the control of the PBMs.
Focusing on Financials
Mary Walsh, owner of Exton Pharmacy knew that her financials were not in a good place so she started paying attention. They started a weekly chart so they could quickly see what was happening and also created biweekly reports to to look at the number of prescriptions and their gross profit per prescription. Clarity into her financials gave her the knowledge she needed to make smarter and more informed business decisions.

From Almost Closing to the Best Year Yet
Tanglewood Drugstore was on the verge of closing. They decided to give it one more shot and joined PDS. With PDS, they were able to focus on building a better team culture with more engaged employees, which has given them a stronger and more stable foundation to run their business on. They also greatly increased their Sync program which has vastly improved work flow. With the guidance of PDS, these changes led to their best year yet.
Giving Employees Ownership
When owner, Adam Wheeler was asked what he did differently now that he was with PDS, his answer was that he stopped micromanaging. By letting his people do more and have more ownership, he found that they were more invested. It also allowed him to the time and ability to concentrate on running his business.

“You must fight to succeed, but if you‘re willing to be open-minded, be aggressive and make calculated risks, there’s so much opportunity out there. There‘s a lot of success to be had, but you must have a team on board that believes with you. In an industry like this where it’s so easy to fail, there‘s not much room for negativity. You’ve got to look at the opportunities and go after them because they are out there no matter where you‘re at in your business. You’ve just got to be willing to dig for them and then put in the work.”
Growth Through Compounding and Provider Relationships
Last year, North Century was compounding 60 prescriptions per month. Currently, they are approaching 300 per month. Year over year, they are pushing a 400% increase in compounding growth alone! For the first few years, Easton spent a lot of money and energy advertising compounding, but he was still battling being the new guy with out-of-state mail-order competitors. He had to shift his strategy, so he started focusing more on building provider relationships.
He is intentional about the relationships. He has been evolving his strategy, making the most of every meeting and not leaving anything on the table.
Diving in with the Community
One of Easton’s favorite parts about what he does is his ability to involve North Century in the community.
Easton supports the community is by utilizing extra space. “Instead of trying to fill it up with stuff that wasn’t going to sell routinely, I have local artists and vendors put their artwork on our shelves, so it basically gives them a storefront.” Easton runs this on consignment and gives 90% back to the vendors.
Beyond that, Easton has focused on bringing the community into North Century through value and information. They host seminars to help educate patients on important topics such as weight loss and CBD, among other things. It’s just another way North Century has been able to connect with the community and show why independents matter.
Working Together on a Common Goal
Easton was adamant that having a team that’s aligned and working towards a common goal is why North Century continues to see success. He also mentioned the PDS mindset of incentivizing and engaging your team is at the core of what they do.
He built a bonus structure that’s worked very well. “As long as we meet our set gross profit per prescription goal, they receive their max bonus. Then I have it tiered down. I do a 90-day rolling average, too, because many valuable prescriptions are filled as 90-day supplies.”
“I love being creative. I love marketing. I love being able to come up with an idea in the middle of the night and then talk about it with my team the next day.”
“I’ve talked to lots of colleagues and realized that we incentivize differently than most. We set the bar high and then compensate unselfishly. I don’t think I am over the top about it, and I’m glad to do it for them.”

Hitting the Ground Running with SmartDispense
MedTime learned about SmartDispense at PDS Super-Confernece in 2018 and knew how important it was. They hit the ground running with somebody dedicated to it in each of the stores. When they first started at MedTime, they were at $8.50 per prescription. Then within three to four months, MedTime was up to $18. One month they were up to $21!
They focused on the PDS dashboard to create buy-in from the team. They emphasized the importance of clinical value for the patient to their team and to the Pharmacist in Charge. They also explained the importance to the patient, as well as the physician. As the program has evolved, MedTime continues to get more in-depth with the physicians as their relationship builds.
Learning & Growing from Advanced Leadership
Owners, Tamer and Elizabeth Ibrahim had a positive experience after attending the PDS program, Advanced Leadership (ALP). The first module taught them all about awareness. It helped them become not only more aware of themselves, but most importantly, the other people in the room and how to lead them. Module two focused more on leadership style and coaching, how to have a critical conversation with an employee, and how to get an agenda done.
“I think the leadership style was really something that stuck out to me. It’s important to have different leadership styles, all mixed to create the perfect team. Just having a better understanding of how people react to things based on their personality,” Elizabeth mentioned.
“There was a mindset shift of working on the business rather than just being in the business.”

A shift in Mindset
Nirmit Patel, Owner of Whiting Pharmacy, details his first three years in business as just a pharmacist who opened a pharmacy. With the help of PDS, Nirmit describes his next two years as “a business owner who just happens to be a pharmacist.” Joining PDS completely shifted Nirmit’s mindset. He’s now focused on growth and making his business as profitable as possible. Many business owners, albeit, have a right to complain, yet within the PDS network, problems are turning into solutions.
Growing with CBD
After attending the 2019 PDS Super-Conference, Nirmit was ready to sell CBD to his patients. He saw this as an opportunity to present a healthy alternative and grow his sales. “We picked up so many different ideas from direct marketing to having bag stuffers and doing seminars around the town. We just had to communicate with the individual patients. If someone came in with aches and inflammation, instead of giving them Tylenol or Advil, we started talking about CBD.”
Nirmit had his employees go through training to make sure they were educated and well-versed on such a popular product. He wanted to make sure they were aware of different dosing and methods of taking the product. It paid off, as the pharmacy went from non-existent sales to averaging $12,000 – $15,000.
Mitigating DIR Fees
Ninety-nine percent of Whiting’s clientele is Medicare Part D. He was filling more prescriptions than ever, which meant more revenue, but he also needed to make sure he kept his DIR fees as low as possible.
Since it’s impossible to eliminate them, owner, Nirmit, was laser-focused on making sure he kept as much of his hard-earned dollars as possible.
“We just needed to understand it all. Some PBMs are a flat DIR fee, and some are GER or BER. For us, the most important thing was understanding that, and then
making sure that our team, from technician to cashier to the delivery driver, are aware and understand what the DIR fees are and the importance of keeping them under
control.”Nirmit’s pharmacy is an Amplicare and FDS customer. They use these platforms to keep their scores within the top five percent. “That’s another thing we are very, very proud of. We’re keeping our scores very high and keeping the DIR fees at the lowest we possibly could,” says Nirmit.
“You can still have positive growth in your business. It really comes down to what you’re willing to do and if you’re willing to learn new tricks. Pharmacy is still a very great business to be in. It’s still a very profitable business as long as you’re willing to make changes.”
“CBD was huge for us. We went from almost nonexistent sales to now we’re averaging about $12,000 to $15,000 in CBD sales. It was a huge jump for us, going from zero to that number.”

Creating a Vision to Increase Employee Engagement
Mark Aurit along with his two daughters quickly expanded from one location to four. With the growth into more locations along with the growing number of employees, there was a great need to restructure and put some key procedures and people into place. The Gateway team credits PDS for helping put a framework together that helps them to develop values and vision. They have confidence that they are all going in the same direction with the same goals in mind.
PDS Trainings Lead to Real Success and Real Revenue
Gateway has sent multiple employees to PDS training programs such as Advanced Leadership (ALP) and SyncRx+. These programs led to the creation of a nutrient depletion program that has become very successful for Gateway. Now they feel confident about offering pharmaceutical-grade vitamins to their patients. They’ve sold 3,432 units, which brought in additional revenue of $30,000 to the store in 2019. By finding key people at each location that were passionate about the program and sending them to PDS training programs, they were able to create a new source of revenue.

It was a struggle initially to get four pharmacies on board, but now we’ve got the right people in the right spots, and they’re excited about what we’re doing and where this is going.
